Thank you for being willing to foster an animal!

Short term fosters enable us to rescue more animals from the shelters. It is necessary to keep the foster animals in an area away from your family pets at the beginning to protect your own pets, unless the rescue has already been vetted.

Please note that you must be over the age of 18 to be considered to foster one of our rescues.

Thank you for agreeing to be a foster parent for Interstate Partners for Animal Welfare (IPAW). Please check the box next to each statement after reading it. Note that checking the box will serve as initials until a scanned copy with signed initials can be obtained.

I agree to provide food, water, shelter, exercise and the best possible humane treatment.

I agree that my services as a foster care provider are given on a voluntary basis. There will be no pay or compensation of any kind given by IPAW.

I agree to contact my foster coordinator by phone, e-mail, or in person to provide updates on the progress and well-being of the foster animal(s).

I agree to contact my foster coordinator immediately if the foster animal(s) develop any behavioral or medical concerns or if the animal becomes lost.

I acknowledge and agree that IPAW retains full legal ownership of the fostered animal(s) and that the fostered animal(s) remains the property of IPAW.

I agree that the ultimate disposition of any animal(s) in this foster care agreement will be determined solely by IPAW. If the return of the animal(s) to the IPAW is requested, it will be done promptly.

I agree that any breach of conditions of this foster care agreement may result in immediate termination of this agreement. IPAW will take immediate possession of the fostered animal(s).

I agree that IPAW provides no guarantee as to the health of the fostered animal(s) and that the fostered animal(s) may have

I agree to keep the animal(s) at the property listed above and agree not to transfer or move the foster animal(s) to any other location without the express written consent of an IPAW representative.

I agree that I will be the primary caretaker of the fostered animal(s) and agree to ensure, and take full responsibility for, the health and well-being of the fostered animal(s).

I agree to comply with all local and state laws and ordinances relating to the care of domestic animals.

If for any reason either party decides that the foster situation is not working out, I will return theanimal(s) to IPAW.

If I decide to adopt the foster animal(s), with the approval of IPAW, I agree to complete the adoption process and pay for the adoption fee.

I agree to allow an IPAW representative, or a designee, into my home to perform an inspection before my application to foster is approved.

I agree to let IPAW into my home to perform reasonable, routine inspections to ensure that the animal(s) I am caring for are being kept humanely. If IPAW does not feel, for any reason, that the environment is proper for the animal(s), they may repossess the animal(s) at any time.

I acknowledge that IPAW has fully apprised me of any existing medical condition(s) of the animal(s) and has given me written instructions on the animal(s) treatment, housing, etc. due to such medical condition(s). I agree to follow such instructions and to contact IPAW with any questions or clarifications regarding such medical condition(s).

I agree to contact IPAW immediately should the foster's medical condition(s) change or worsen to receive further instructions regarding obtaining veterinary care for the foster animal(s).

I expressly agree to release, discharge, indemnify, and hold IPAW harmless for any and all claims and damages caused by the foster animal(s) directly or indirectly, to my personal property or pets, as well as other persons, or the property of others, while the foster animal(s) is in my care.

I will not engage in or allow any others in my household to engage in any intentionally harmful or negligent care of the animal(s).

Intentionally harmful or negligent care can include not providing access to adequate food, water, shelter, and/or medical treatment; not providing the animal with conditions that are sanitary; maintaining, caging or tethering the animal(s) for an extended period of time, or in inhumane conditions or in insufficient space; beating or taunting the animal(s); acting in a way that might lead the animal(s) to develop or manifest aggressive behavior, or failing to take appropriate action to prevent aggressive behavior.

I am aware that dogs and cats respond to situations differently, that such responses may be unpredictable, and that IPAW makes no claims, representations, or warranties, either express orimplied, as to the behavior or temperament of the fostered animal(s).

I acknowledge that IPAW is not liable for any veterinary expenses of the animal(s) should the foster care provider seek veterinary treatment from any outside source other than IPAW. Such veterinary care is not authorized by the terms of this agreement.

I agree that my own animals must meet the following requirements prior to my foster placement:

Canines must be immunized against Canine Distemper, Canine Parvovirus, Parainfluenza, Hepatitis, Bordetella, and Rabies. They must also be free of parasites.
Felines must be immunized against Feline Panleukopenia, Rhinotracheitis, Calicivirus, Feline Leukemia, and Rabies. They must also be free of parasites.

I recognize that in handling foster animal(s) there exists a risk of injury including physical harm caused by a foster animal. On behalf of myself, my heirs, personal representatives, and executors, I release, discharge, indemnify, and hold harmless IPAW and its volunteers from any and all claims, causes of action or demands, or any nature of cause connected with my foster care agreement.

I assure that the information I have provided in this agreement is true and correct. I understand that falsification of any part of this agreement will result in termination of this agreement.
